REFERENCE
TO THE OFFICIAL JOURNAL OF THE REPUBLIC OF FRANCE
(JO of the 25/01/1978)
Decree No. 77-1558
of 28 December 1977 (Official Journal of the Republic of France of 25
January 1978) of the French Ministry of Health and Social Security requires
the declaration of the formulae of cosmetic products and toiletries according
to a regulatory form, whose characteristics were defined by the by-law
of 15 February 1978.
A standardized presentation is required for the purposes of legislation
and classification.
One envelope must contain the formula of a single product. The envelope
is kept sealed, except in the case of an emergency.
Only the manufacturer who possesses the complete formula of the product
must declare this formula to our department.
As soon as a new product is released onto the market (or following a change
of brand name, trade name or range name), you must send us the "CERFA"
No. 61-2128 ENVELOPES (that can be obtained from the printer, SAGIM, see
address below), after completing the following information:
Frame 1:
Trade name and brand name of the product, corresponding to the product
labelling.
Frame 2: Name and address of the establishment manufacturing,
packaging or importing the product, telephone and fax numbers.
Frame 3: Category and use of the product (part of body
concerned by the product: hair, nails, feet, face, etc.).
Frame 5: Date of establishment of the form.
THESE ENVELOPES MUST
CONTAIN A "CERFA" FORM (or several forms depending on the formula)
completed by computer or typewriter (or on a folded A4 format sheet of
paper), which must comprise:
- The same information
as that contained in frames 1, 2, 3 and 5 of the envelope.
- The names of the ingredients, respective chemical names, their percentage
in the composition, and pH, as well as the presentations and capacities
of the various commercial packagings.
These forms must be
accompanied by a summary list, in duplicate, of the envelopes submitted
and all of these elements must be sent in a single dispatch, by registered
mail with receipt, to the Cosmetics Bureau of the three Poisons Centres
(LYON, MARSEILLE, PARIS) designated by the by-law of 27 January 1978):
